Could Carnival have avoided the Triumph cruise nightmare?

It was the vacation from hell for thousands of passengers stuck on-board the Carnival Triumph. A fire knocked out the ship’s power, which meant no air conditioning, no lights, little food and water… and no flushing toilets. Conditions were so bad it was nicknamed “The Poop Cruise.” Now CNN has learned that the ship set sail with only four of six generators operating and knew of a generator fire hazard across its fleet of ships.
